Location: New Barnet, Hertfordshire
We are currently looking a Care Coordinator to help in the efficient running of a domiciliary care office looking after elderly service users.
To assist in the day to day operation of the home care business.
To participate as required in multi disciplinary assessment of service users and subsequent planning of service user care.
Define special care needs and monitor and review.
Liase with professional care organisations in respect of developing total package of care for service user.
Report to Branch Manager weekly activity levels for service users.
Participate in training.
Provide written reports on complaints, accidents and incidents.
Health and safety.
Review meetings.
You will have previous experience in a Care Coordinator or Care Supervisor or Team Leader role with a minimum standard of NVQ 2 or 3 in care Office based role provisionally with travel to service users when required.
My client is a National Domiciliary Care Agency which operate on a franchise basis.
