The Role:
In the Care Coordinator role you’ll report to the Deputy Manager and will be primarily responsible for supporting the Deputy Manager to ensure that the service is ran at a high standard within the CQC guidelines at all times. You will be tasked with completing various managerial duties such as; audits, care plans, supervisions, safeguarding matters etc. You will work with your office and field base to ensure that the day to day running of the business is as smooth as possible and the customers are safeguarded whilst being provided with person centered care.

Specific tasks include:

  • Liaising with client families, health professionals and council authorities
  • Supporting and assisting the Home Manager in auditing care plan documentation, to ensure best practice
  • All things that are related to scheduling/rotas

    The Candidate:
    To be the right person for Care Coordinator role you will need direct experience of supporting adults with learning difficulties, accurately documenting care plans, risk assessments and good knowledge of mental health act.

    We also need you to have prior experience of completing administration associated with social care as well as liaising with families and medical professionals.

    It’s important that you also have the following:

  • A NVQ level 3 or above in Health and Social Care
  • Fantastic communication skills
  • Experience previous within a similar setting as a senior member of staff

    You’ll also need to be the following:

  • Someone with a pro-active attitude
  • Self motivated, flexible, caring and organized
  • Driven and ambitious

    The Package:
    The basic salary for the Care Coordinator role is up to £30,000 per annum.

    There are further payments you’ll receive as a result of your on-call work which will amount to £0 per annum.
