This is a fantastic opportunity to join for a well-established learning disabilities provider as a Referrals and Assessment Manager. As the Referrals and Assessment Manager you will be establishing new areas of business for the organisation and to drive development. Reporting to the Development Manager, you will be instrumental in working to maintain and establish new social worker and care manager relationships and develop a robust referrals pipeline in line with area sales targets for new business.
As the Referrals and Assessment Manager you will be responsible for providing a positive and seamless journey for people requiring learning disabilities support. Also, communicating with families, Care Managers and health professionals to offer reassurance and proactive management to put in place a holistic package of support.
You will be required to undertake assessments to an excellent standard, propose service hours and produce detailed, outcome-focused support plans and risk assessments. Working alongside operational area colleagues, you will lead the implementation ensuring a robust and person-centred focus to all systems and processes.
As the Referrals and Assessment Manager you will need to have experience in developing new business and strong operational expertise within the Learning Disability, Mental Health. Also holding a NVQ level 4 or above in health and social care or a similar qualification.
