We currently have a fantastic opportunity for a Team Coordinator to support clients with learning and physical disabilities based in Edgware, North London.
You will be working within a small team, managing a portfolio of clients and support workers, providing support to service users with various conditions within a supported living environment.
There will be many varying duties that you would be required to undertake, including:
– Multi-disciplinary assessment of service users and subsequent planning of service user care
– Define special care needs and monitor and review
– Report to the proprietor the weekly activity levels for service users
– Provide written reports on complaints, accidents and incidents
– Review meetings
– Awareness of CQC minimum standards

They have contacts with the local councils and are at the forefront of care in the areas they cover.
Experience in social and or health care services is essential as is the experience of managing a team. Ideal candidates will have, or be working towards, NVQ Level 3 in social care or extensive experience in the learning disabilities or physical disabilities management field.
